If you are going to be mailing the records and authorization, would you mind sending us more than one originally signed authorization? The medical providers usually take copies, but you never know when we might be required to provide an original. Thanks again!!!

(1) Please, please make sure that Ms. Schenkel, Mr. Ashton, et.al. know that my mother’s name is KATHLEEN Gagne (and not Katherine).

(2) All the mail I ever sent to my mom at Stewart-Marchman ACT was sent to an address in Daytona Beach, Florida on Red John Drive. I’m not sure where Ms. Schenkel found the Deland address referenced in her email.
